1/8/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. Ashiyah indicated to QMHP-C that she was having a good day. Mother reports Ashiyah struggles with controlling her anger as evidenced by her slamming doors in the home when she cannot have her way with behaviors lasting about 30 minutes to an 1 hour in duration. 2. Behavior: Ashiyah actively participated in the session by cooperating in discussions and expressing her feelings appropriately. Ashiyah indicated that she will continue to utilize alternative measures as a preventative measure to being sporadic and being aggressive toward her mother and peers with behaviors lasting about 20 minutes-40 minutes in duration. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C reviewed with Ashiyah on ways to express her feeling when she is depressed. QMHP-C reiterated several coping skills that would continue to help build positive relationships with others. QMHP-C modeled effective ways to handle her triggers that causes her anger, rage, and disrespectful behaviors toward others. QMHP-C engaged Ashiyah in several scenarios that would support her in any situation. 2. Intervention: QMHP-C processed with Ashiyah on calming skills that would support her to avoid reacting negatively and being explosive when interacting with others. QMHP-C talked to Ashiyah about the importance increasing awareness of accepting her responsibilities and learning from mistakes. QMHP-C engaged Ashiyah in a discussion surrounding the importance about being mature and responsible in the home, school, and community setting. QMHP-C utilized a therapeutic role play to educate Ashiyah on the importance of being calm and humble while an adult is giving advice. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah reported that she has struggled with outburst and being quick tempered. 2. Response: Ashiyah shared that she has been utilizing coping skills evident by walking away and calming herself down before escalating from 1-10 instantly. Ashiyah indicated that she will continue to utilize alternative measures as a preventative measure to being sporadic and being aggressive toward her mother and peers. Ashiyah indicated to QMHP-C that she attempts to utilize coping skills when she is angered, but they don’t come to her when she needs them.
